As a convenient and educational resource for our clients, we have supplied all necessary forms for submitting samples to Veritas Laboratories including our Chain of Custody form, a guide to assist with sample preservation and correct container usage, and instructions for filling bottles and jars before sample submittal to the laboratory. Chain of custody
Download, print, and fill-out our Chain of Custody (COC) form before submitting samples to the laboratory. Hard copy Chain of Custody forms in duplicate our available in our Sample Receiving area.
Bottle order form
Contact us to pre-prepare bottle kit orders for you. Provide usby email your project name, number of samples, sample matrix, and the analyses for each sample, and we'll do the rest. Please also indicate when you would like the sample coolers delivered to you or ready for pick-up at the laboratory.
CONTAINER AND Preservation and guide for aqueous samples
We've provided a list of common aqueous analyses and the corresponding method, holding time, container size/type, and preservation. Please contact us to if you want us to set-up a bottle order ahead of time. Bottle kits can be pre-labeled with your Company Name, Project Name, Sample ID's and Analyses/Preservation. We make it easy to just fill the bottles and return them to the laboratory!
